Does the open-ear design include ANC?

No. Pebble Aura does not seal the ear canal and does not provide active noise cancellation.

Can the TV speakers stay on?

That depends on whether the TV supports simultaneous speaker and Optical/AUX output. The transmitter does not provide audio pass-through.

Are Auracast hearing aids included?

No. Compatible Auracast hearing aids can join the TV broadcast, but they are not included.
